Note: Within 24 hours of Rating an Incident that requires investigation, the Head of Safety or the designated authority with appropriate resources will need to form the Investigation Team and set a due date for completing the initial investigation report. 

Fill the first information report form giving the details of the exact location and description of the incident, the extent of damage (in terms of cost) and the immediate action taken. If there were Victims, select Yes, and add the details of the victims and the body injuries, if any.

While adding a victim, you can specify the type of victim – whether an employee, contract worker, or visitor. Giving details about the injury helps in the investigation process, identify potential root causes and implement corrective actions to avoid recurrence in the future.

In Safetymint Incident management module, you can add up to 5 victims in 1 incident form, detailing the injuries and immediate treatment given for each individual victim.
