Within 24 hours of reporting an incident, the Head of Safety or the designated User with appropriate rights is required to classify the incident based on the rating matrix as shown below. Incidents are rated as Minor, Serious or Fatal based on a score arrived at by a combination of the likelihood of occurrence and the Severity potential (as shown below).


Click on the appropriate radio-button within a cell. The color code is a guide to the severity of the incident. Based on the rating, the need for an Investigation may be decided accordingly.

 

If you believe an investigation is required, select Yes and submit the rating. 


The investigation process will then start by following the 5-step investigation process as follow ...

 

Step 1: Prepare Incident First Information Report (FIR)

Step 2: Form the Investigation Team

Step 3: Identify Root Causes

Step 4: Recommend Appropriate Actions and submit for Approval

Step 5: On Approval, review and close on Recommended Actions



 

If an investigation is not required, then select No and submit the rating. The incident card can be closed by recommending and assigning appropriate corrective actions.
