A safer, healthier and sustainable workplace is when everyone participates and is closely involved in the betterment of one another. Recording and reporting incidents in a timely manner are important and integral behaviours of safety champions.


At any point of time, click on the [+ New] button on the top right of the header section and select [Incidents] from the dropdown list. 


You can also access this form from the right side of the Incidents screen – as shown below. 


Follow these steps to report an Incident.

Step 1:


Start by selecting the type and category of the Incident. Categories are grouped under these types of Incidents – 1) Near Miss, 2) Lost Time Injury, 3) First Aid Case, 4) Fire Hazard, 5) Environmental.

You can drill-down under a few types to select the appropriate incident type. For example, you will find the following categories under the Lost Time Injury type …

- Fatality
- Medical Treatment Case
- Restricted Work Case
- Reportable Accident

Note: You can create and manage your own categories and types from the Settings section of the Admin module. We have pre-populated a few globally used terms for your convenience. You can enable, disable, remove or rephrase any of the existing terms to suit your organization’s preferences. 

Step 2:


Describe your Incident in a few words that can help the reader to understand and comprehend the situation. It is best to seek help in case you are unable to find the right words to express the meaning clearly.

In the mobile app, we have provided speech to text capability (in English Language only). Feel free to dictate the details of the incident instead of typing.



Step 3:


Select the exact location, by selecting the Site, Division, Department and/or Unit from the dropdown. It is not just enough to select the Site – as the site may span a larger area. Narrowing down to the exact location helps in making better-informed decisions.


Step 4:


Select the work shift during which the Incident occurred. This field is optional.


Step 5:


Select the date and time when the incident occurred. It is good to report the almost exact date and time of the incident occurrence. It is best to seek help from witnesses with first-hand information to report the date and time.


Step 6:


Attach pictures of the Incident, if any. Please ensure that you are reporting responsibly. Upload sensitive images with caution and with due approvals from stakeholders in legal and HSE.




Step 7:


Submit the form. Once submitted, the Head of Safety and the designated users of the location are notified. The Head of Safety will then rate the Incident based on the details and take further steps to initiate an Investigation or Close the same.
